- Print the library card application found in mplservice email.
- Delete that email!
- Look up the patron in Workflows.
- Verify their identity using birthdate. Make sure to check for nicknames such as William/Bill.
- If the applicant has a verified current account
- Update their account with information provided in the application.
- Reply via email to let them know they have an active account. Provide them with their barcode and pin number. Remind them they can use this information to place holds, and use Curbside Service and access databases. **
- Shred this application once patron has been emailed.
- If the applicant is an adult with no current account
- Create a library card using the basic steps in Workflows. **
- Put a note in the record, including your initials and date, and on the printed application, stating that an original card was mailed to them.
- Mail the card to the patron at address provided. Include the two small green forms-one with MPL info, and the other with “New Library Card” info. **
- The completed application should be placed in the drawer with other applications needing to be re-checked by staff, and then saved.
Note: We do not mail cards to underage applicants.
Per our recent discussions, a parent may apply at the curbside table for a youth or young adult card for their child.
**If patron wants to provide $2.00 for a replacement card, we will bill their account, and mail a replacement card.

**If you know how to process the envelope for mailing, please do so. If you are not familiar with the mailing procedure, please ask for help.
